Consultant
David is a metallurgical engineer with 29 years of experience in
a broad range of areas. His expertise includes forensic metallurgy,
litigation, failure analysis, life assessment, fitness for
purpose assessments, repair methodologies, manufacturing processes
and risk management of pressure vessels, piping and rotating
equipment.
David has investigated a number of major incidents and near major
incidents (i.e. with a loss or potential loss of many millions
of dollars) in industrial plant to determine the root cause,
how to repair the plant and how to ensure adequate plant life.
These have included blown out petrochemical reformers, major
gas leaks on offshore platforms, failed hydro power station,
failed ships engines and numerous fatal aircraft accidents.
He has also acted as an independent expert in a number of
litigation cases.
